We would like to tell you about an important change in the way your wound care service will be delivered in the future.
How is my wound care service changing?
Your current wound care service is being provided by practice nurses at your local GP surgery. This service will stop on 30th April 2020. This is because the service is being transferred.
The new wound care service will be provided by Central Cheshire Integrated Care Partnership, also known as CCICP.
The new service will begin on Monday 4th May 2020, and will be sited at the Victoria Infirmary Northwich, also known as VIN.
How will I transfer to the new service??
You will be referred by your Practice Nurse to your Care Community Team.
The Care Community team will be able to access your electronic clinical records and make your referral appointment for you.
Who will deliver the new wound care service?
The new service will be delivered by specialised wound care nurses from your local Community Nursing Team who are managed by the CCICP.
What do I do next?
You do not have to do anything as everything will be arranged for you. However, if you do wish to discuss these changes, please get in touch.
