I-CARE & Share programme

The I-CARE & Share programme is being administered through NHS Cheshire and Merseyside and its partners. It seeks to further the six ambitions of Palliative and End of Life Care: A national framework for local action 2021-2026.
These ambitions are:
- Each person is seen as an individual
- Each person gets fair access to care
- Maximising comfort and wellbeing
- Care is coordinated
- All staff are prepared to care
- Each community is prepared to help
When used as recommended the I-CARE & Share package meets the requirements for:
- Gold standard End of Life care
- Palliative care
- Primary Care Network DES for care home residents
- Complex patients with long term conditions
- Use by doctors, nurses, pharmacists and care coordinators
- Business intelligence capture
- Data sharing via shared and summary care record
- Electronic Palliative Care Coordination System (EPaCCS)
Phase 1 involves spread and scale up across our nine Places to General Practices and community services.
Phase 2 involves development and involvement of secondary care, specialists and hospitals.
